Order Description
This is a 4yrs old girl with asthma attach. She is on albuterol puff. On O2.
These are my five nursing diagnosis that am working on:
1) ineffective breathing pattern
2) impaired gas exchange
3) caregiver role strain
4) deficient knowledge
5)ineffective airway clearance
My instructor would me to implement four nursing action, four rational and four evaluation for each nursing diagnosis. Measurable,specific, attend able,time and realistic short term and long term goal for each nursing diagnosis. To Use two nursing journal and any other not older than five years. See upload. Please do not bother about the six five pages. I want each page of the 7th to 11th page for one nursing diagnosis each. please use the nursing diagnosis i provided.
