Individualized Transition Plan

Last week, you completed a transition assessment for a mock student. Using that document as a guide, create an IEP for the mock student. You should use
the Colorado IEP form (located under IEP Review heading: https://www.cde.state.co.us/cdesped/iep_forms). Your final IEP should include all sections,
including the ITP.
