Your ‘Care Plan” will include recommendations for community resources and family involvement. Your recommendations will promote Quality of Life, dignity, self-determination, and happiness, while also tracking progress and accountability.
Case Scenario #1: Coen’s Story
Coen is a 14-year-old boy with Autism Spectrum Disorder. He lives with his mother in Toronto, Ontario. Coen and his mother moved to Toronto one year ago from a First Nation reserve in Northern Ontario. His parents are currently going through a divorce, and Coen currently does not have any contact with this father. His family is English speaking.
Coen is in grade nine at his local public high school. His current Individual Education Plan (IEP) is primarily focused on academic goals. He attends school most days but will often complain about feeling sick, hoping he will be allowed to go home. He is described by his teachers as impulsive and always on the go, which makes it hard for him to stay focused and in his seat. Despite some of these challenges, he does well in mathematics, and is enjoying learning to cook in his hospitality class.
Coen has a moderate developmental delay. His mother has expressed difficulty accepting his diagnosis and feels overtime he will “outgrow it” and go on to university. His mother is concerned about his lack of peer relationships and would like for Coen to make friends, gain confidence, and learn how to socialize with his peers.
